Overview:

In the context of Child-centered, Family-focused practice to promote Permanency, Safety and Well-being, and to reduce length of stay in foster care, the Social Worker ensures effective and efficient case management and clinical services to the adolescents in care and their families/significant others. Social Service delivery is to be consistent with Good Shepherd Services philosophy and within city and state mandates.

Reports to: Director of Social Services

Location: 337 E. 17th Street, New York, NY 10003 (On Site)

Hours: 35 Hours/Week; Exempt

Salary Range: $61,000 - $72,000 Annually based on Licensure and relevant experience

Major Duties:

  • Perform intake and assessment interviews.
  • Complete written psycho-social assessment of participants and family/significant other.
  • Participate in all interdisciplinary treatment team/service plan review/20 Day, FTC conferences, and Court to ensure consistent service planning.
  • Provide short-term treatment to participants, families and significant others.
  • Assist with stabilization, respite and crisis management.
  • At intake, begin the process of identifying permanent connections and formulating discharge planning.
  • Formulate a written recommendation based on participant/family and team input.
  • Promote permanent connections, including reunification, foster boarding home placement, adoption and alternative living arrangements.
  • Lead two groups per week
  • Assist in youth and family transitioning to next phase of their care post discharge.
  • Collaborate with referring and community agencies to advocate for the needs of participant and family.
  • Integrate all written components of the diagnostic study for transmittal to the referral agency, ACS and OPA.
  • Participate in meetings both weekly and as needed with youth development staff, school and other staff for case collaboration and planning.
  • Participate in weekly individual and group supervision as indicated.
  • Attend internal and external meetings as indicated.
  • Conduct home visits and escort youth to court and other appointments where indicated.
  • Maintain mandated paperwork and recordkeeping in compliance with ACS, OCFS and GSS PEP Dept.
  • Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-education groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care in the department/program to create a non violent and trauma sensitive culture.
  • Promote and model the seven Sanctuary Model commitments to craft a non-violent and trauma-sensitive culture.
  • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
  • Assist in program events: including coordinating setup and clean up.
  • Perform other duties as assigned.

Qualifications:

  • Masters degree in Social Work and licensure (LMSW) or licensure ready required (within the first 6 months of employment).
  • Experience working with adolescents and families required; foster care/ACS experience preferred.
  • Bilingual English/Spanish preferred.
  • Able to establish working relationships with clients in need of supportive services.
  • Proven competence in written, verbal, organizational, and prioritization skills.
  • Knowledge of Connections and Microsoft Office Suite.
  • Ability to work a flexible schedule when needed including some evenings, weekends and holidays.
  • Able to use public transportation when attending various meetings or making home visits.
  • Have a dedication to work from a strength-based, youth development and trauma informed perspective.
  • Have a dedication to ACS principles to promote permanency, safety, and well being.
  • Have a commitment to work from a strength-based, youth development and trauma-informed perspective.
  • NY State Driver License Preferred
