\n \n\n \nShift : Mon – Fri 8:00-4:30/ On call weekend commitment Monthly/Quarterly
\n \n\n \nLocation: Kansas City Area – Service area can include Leavenworth, Johnson, Wyandotte, Franklin, Douglas and Miami Counties in Kansas; Jackson, Platte, Clay, Cass Counties in Missouri
\n \n\n \nThe role you’ll contribute:
\n \n\n \nThis position is accountable for providing professional nursing activities as identified and planned by the R.N. Case Manager in the Patient Care Plan to maximize function within the home restoring health or assisting a peaceful death. The L.P.N. is supervised in the home by the R.N. as often as necessary, but at least every sixty days

\n \n\n \nQualifications
\n \n\n \nThe expertise and experiences you’ll need to succeed:
\n \n\n \nEDUCATION AND EXPERIENCE REQUIRED:
\n \n\n \n·Graduate of an approved school of nursing
\n \n\n \n·At least one (1) year of nursing experience in a hospital setting or community setting involving direct nursing care
\n \n\n \nEDUCATION AND EXPERIENCE PREFERRED:
\n \n\n \n·Previous home health experience.
\n \n\n \nLICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
\n \n\n \n·Current license to practice as a professional nurse in the State of Kansas and Missouri.
\n \n\n \n·Maintain CPR certification.
