This vital role ensures the delivery of compassionate care to our patients and their families during non-traditional hours, maintaining the quality and accessibility of hospice services when it matters most. Key Responsibilities for Hospice RN After Hours Provide direct patient care according to established hospice care plans during after-hours shifts to ensure patients' comfort and dignity. Respond promptly to patient and family needs through phone consultations and on-site visits as necessary. Collaborate with interdisciplinary teams, including physicians and social workers, to ensure comprehensive care coordination and support. Document patient interactions and interventions with accuracy and timeliness, ensuring continuity of care. Educate families on symptom management, medication administration, and end-of-life care processes. Schedule and Shift Details This position covers non-traditional working hours, including nights, weekends, and holidays to meet hospice care needs beyond regular business hours. The working hours are from Monday to Thursday 5p to 8a. The position is open to Northumberland, Snyder, Union, Montour, Columbia, Lycoming, Clinton, Sullivan. Qualifications and Requirements Current and valid Registered Nurse (RN) license in the state of employment. Minimum of 2 year experience in hospice, palliative care, or a related nursing field preferred. 1 year of experience in ANY of EMR, Other EMR and TruChart EHR Certification in hospice and palliative nursing is a plus. Reliable transportation and the ability to travel to patient homes as required. Strong clinical, communication, and organizational skills to manage after-hours patient care independently.
