About the Role: Job SummaryResponsible for the provision of bereavement, grief support services to hospice patients/family members and the local community consistent with Hospice policies. Flexible in work hours to meet the needs of the program. Some evening and weekend hours required.
Minimum Job QualificationsLicensure or other certifications: Georgia license in Clinical Social Work (LCSW) required. Maintains current BCLS certification. Certification in bereavement counseling preferred.
Educational Requirements: Masters Degree in Clinical Social Work from a school accredited by the Council on Social Work Education (CSWE).

Ensures the provision of bereavement services for the hospice program. Assists the Interdisciplinary Team in recognizing and responding to grief/support needs of patients/family members/caregivers. Evaluates bereavement needs assessments from the primary social worker and Interdisciplinary Group. Collaborates with the family and hospice team in developing and updating the bereavement plan of care to include individualized care for a period of at least thirteen months following patient death. Provides bereavement counseling and other interventions as indicated in the bereavement plan of care including but not limited to home visitation, support to Long Term Care residences, individual/group counseling. Provides case management services as indicated including referrals to appropriate community counseling and/or practical needs resources. Ensures delivery of grief support mailings such as condolence letters, newsletters, educational materials. Coordinates with the volunteer coordinator to involve and provide oversight of volunteers in the bereavement plan of care as appropriate.
Serves as a resource to the hospice program and the community in the area of grief and bereavement. Organizes and provides for grief support groups in the community. Organizes and provides for remembrance and memorial services. Organizes and provides other appropriate services such as bereavement camps, grief support groups specific to age and loss, educational events. Recommends appropriate books/media and other resources for the program.
Serves as a resource to hospice staff in the area of work related grief and bereavement. Provides resource support to members of the Interdisciplinary Group as requested and as appropriate. Provides grief education and support to members of the Interdisciplinary Groups as appropriate. Provides appropriate education and training to members of the Interdisciplinary Group who assist with provision of bereavement activities/programming.
Adheres to regulatory and organization/department compliance standards. Manages assigned caseload volumes. Adheres to Physician orders for visit frequency. Participates in performance improvement activities. Completes clinical documentation and records of program activity accurately and timely.
Completes special projects and other duties as requested or assigned, such as: Participates in the development of and adherence to an annual program plan of service delivery. Participates in committee work. Assists in development/revision of policies and procedures. Serves as team leader as requested. May mentor students. Assists in covering on-call/after-hours shifts. Provides hospice primary social work services to active patients. Assists volunteer coordinator with volunteer recruitment and retention.
Participates in the patient consultation/admission process. Assists in determining appropriateness for care. Provides patient/family education of the hospice philosophy, services and coverage. Prioritizes a smooth care transition through strong interdisciplinary team communication.
May perform creative interventions inclusive of assessments, care planning, and applicable client/staff education.
Attends meetings as required by department/organization. Participates in staff meetings, work group/committee meetings, education sessions, etc.
Participates in clinical and community outreach activities. Provides patient/family/community education. Assists with patient consultations/admissions to ensure a smooth initiation of care. Promotes a positive public image of the hospice program.
May provide services specific to full LCSW licensure. May supervise Masters level Social Work interns per applicable law and regulation. May complete competency evaluations for initiation of involuntary mental health hospitalization per applicable law and regulations. May bill for services as per applicable law and regulation. May provide supervision toward LCSW licensure per applicable law and regulation.
