Cleveland Clinic Home Care’s East 2 Team provides exceptional home health services across Upper Summit and Lower Cuyahoga counties, serving communities such as Twinsburg, Hudson, Macedonia, Northfield, Stow, Cuyahoga Falls and parts of Bedford and Solon. In this role, you will deliver high-quality nursing care to patients in the comfort of their own homes—performing assessments, providing complex treatments and offering education to help prevent hospital readmissions. Following patient visits, you’ll work remotely to complete documentation, coordinate care, schedule visits and collaborate with the interdisciplinary team. Cleveland Clinic Home Care offers a comprehensive, individualized orientation program and eligibility for student loan repayment, ensuring you have the support needed to succeed.

A caregiver in this role will work days from 8:00am to 5:00pm.

A caregiver who excels in this role will:  

• Provide direct nursing care. 
• Perform accurate and complete admission assessments and reassessments within a specified time frame.  
• Assess patients for safety risks, abuse and neglect.  
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.  
• Assess, document, prioritize and manage patients’ pain.  

Minimum qualifications for the ideal future caregiver include:   

• Graduate from an accredited school of Professional Nursing 
• Current state licensure as a Registered Nurse (RN) 
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross 
• Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter 
• Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter 
• One year of current experience as an RN 
• One year of experience in an acute Medical/Surgical or Home Care setting 

Preferred qualifications for the ideal future caregiver include:   

• Bachelor’s of Science in Nursing (BSN)

Physical Requirements:

• Requires full range of motion, manual and finger dexterity and eye-hand coordination.
• Requires standing and walking for extensive periods of time.
• Requires corrected hearing and vision to normal range.
• Requires some exposure to communicable disease or bodily fluids.
• Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
• Physical Demand requirements are in excess of those for Light Work.
• Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
• Must be able to read, write and communicate with patients.

Personal Protective Equipment:

• Follows standard precautions using personal protective equipment as required.

Pay Range

Minimum Annual Salary: $69,680.00

Maximum Annual Salary: $111,488.00

The pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set and education. The pay range displayed does not include any applicable pay practices (e.g., shift differentials, overtime, etc.).
