Job Title: Health Educator I

Department: Community Impact-PSE

Reports to: Program Supervisor I

Hours: Full-time, occasional evenings, and weekend work possible

Starting Wage: $21/hr.

Essential Duties and Responsibilities:

  • Implement Programs and Services: Assist in the planning and execution of programs, strategies, and services aligned with grant and organizational goals.
  • Outreach and Recruitment: Conduct community outreach, attend events, and recruit participants for programs, classes, and services.
  • Facilitate Classes and Presentations: Coordinate and lead nutrition classes and provide in-person or virtual presentations to community partners and participants.
  • Client Engagement and Intake: Conduct client intakes, build rapport, provide referrals, and follow up with individuals seeking services or resources.
  • Community Collaboration: Develop and maintain relationships with community organizations, providers, and AVPH program staff.
  • Data Collection and Evaluation: Collect and maintain participant demographics, evaluations, and documentation using tracking logs, databases, and referral platforms.
  • Reporting and Compliance: Complete required grant reports, provide monthly updates, and ensure all documentation is organized and submitted timely.
  • Technology and Communication: Utilize word-processing software, spreadsheets, and referral platforms like Unite Us and One Degree; communicate effectively both verbally and in writing.
  • Training and Professional Development: Participate in all required trainings, webinars, staff meetings, and reflective supervision.
  • Community Meetings and Committees: Attend and contribute to community meetings, committees, and collaborative initiatives.
  • Resource Sharing: Identify and update community resources; share relevant information with team members to improve service delivery.
  • Administrative Support: Support special projects and perform administrative duties as assigned by the Program Supervisor I and/or Associate Director.

Experience & Requirements:

  • Bilingual required in order to interview
  • Must hold a bachelor’s degree in nutrition sciences, Health Education or similar degree, or a minimum of two (2) years of experience as a health educator in underserved communities.
  • Experience working with underserved communities and familiarity with community resources.
  • Meet basic health (e.g. TB Test and COVID-19 Vaccination Card) and background check (e.g. Livescan) requirements and physical job standards.
  • Maintain CA driver’s license.
  • Must have automobile or reliable mode of transportation and maintain automobile insurance.

  • Knowledge of departmental policies, procedures, and organizational relationships.
  • Ability to work with interruptions while maintaining positive communication with staff and clients.

  • Safely and effectively uses all equipment necessary to carry out duties.
  • Ability to establish positive rapport with diverse populations, utilizing cross-cultural skills and knowledge of cultural and religious differences, attitudes and practices.
  • Ability to be culturally sensitive, tolerant, and non-judgmental.
  • Ability to maintain confidentiality.
  • Proficiency in Word, PowerPoint, Outlook, and Excel.

Physical Requirements and Working Conditions:

  • Split between in-office setting and community outreach.
  • Working with overburdened, often high-risk clients of low economic and varied cultural backgrounds, living in high-risk areas.
  • This position requires lifting/carrying up to 25 lbs., stooping, bending, squatting, sitting on the floor, walking, running (may be needed)
